Single donation Gift Aid form. Give the completed form alongside your donation or emailed it to admin@stcuthbertsormesby.org.uk
These new Word format forms contain editable fields for you to enter your details; text entry, dates and drop down selection (for eample frequency (Monthly/Quarterly/Yearly). Fill in, print and save a copy for your records.
Security features within Word will mean you need to enable editing to fill in the form alternatively, print and fill in by hand. The older forms are for those who, for any reason, do not with to or cannot use the new form. The Bankers/Standing Order form is for your bank. The Gift Aid form needs to be returned to the PCC Treasurer.
