This new form contains fields for you to enter your details; text, dates and a drop down selection for frequency (Monthly/Quarterly/Yearly). Security features within Word will mean you need to enable editing to fill in the form alternatively, print and fill in by hand. The old form is for those who, for any reason, do not with to or cannot use the new form.
